Judges Form

The first thing the judges see is your judges form. It must make a statement of who you are and why you should take home the title of Miss San Antonio USA or Miss San Antonio Teen USA. We’ve made it simple and easy for you to submit a judges form that looks the part but its up to you to make it sound the part. Please read the instructions below on how your judges form should be completed and sent in, all online.

Once you download the form, type your answers on the document and upload your headshot. DO NOT HAND WRITE YOUR ANSWERS.
Save the form and submit it online where it says ‘Submit Your Judges Form’. The ‘Submit Here’ tab will direct you to a form to fill out and submit your judges form.
